Where heroin once dominated, fentanyl—a synthetic opioid roughly 50 times more potent than heroin—now represents the majority of the opioid supply. The reasoning is ruthlessly practical: more potent substances are easier to smuggle, require less volume to transport, and generate higher profit margins.
A kilogram of fentanyl produces the equivalent effect of 50 kilograms of heroin. For traffickers facing interdiction risk, the choice is obvious. The same principle drove bootleggers during alcohol prohibition to favor hard liquor over beer—more intoxication per unit of volume means more profit per unit of risk.
Modern chemistry has accelerated this process beyond anything available during alcohol prohibition. Fentanyl and its analogues can be synthesized in clandestine laboratories without poppy fields or complex supply chains. Precursor chemicals, often manufactured legally in China or India, can be ordered online and shipped internationally with minimal oversight.
The Price Paradox
Adjusted for inflation and purity, heroin costs roughly one-fifth what it did in 1980, according to Drug Enforcement Administration data. Methamphetamine has followed a similar trajectory. Even as enforcement budgets have grown—the United States now spends approximately $50 billion annually on drug enforcement—street prices have steadily declined.
This defies basic economic expectations. Increased enforcement should constrict supply and drive prices upward. Instead, the market has responded with innovation: more efficient production methods, new synthetic alternatives, and distribution networks that adapt faster than enforcement can respond.
The pharmaceutical industry's role complicates the picture further. The overprescription of legal opioids created a generation of people physiologically dependent on these substances. When prescription access tightened in the 2010s, many turned to street alternatives—creating sustained demand that illicit manufacturers efficiently met with fentanyl.
Public Health Consequences
The potency increase has lethal implications. Fentanyl's narrow margin between an effective dose and a fatal one means users face Russian roulette pharmacology. Someone accustomed to heroin who unknowingly consumes fentanyl-laced product can easily overdose. There's no visual way to distinguish the substances, and street-level quality control is nonexistent.
Overdose deaths have risen accordingly. The CDC reported over 107,000 drug overdose deaths in 2023, with synthetic opioids involved in roughly two-thirds of cases. That's a tenfold increase from two decades earlier, despite—or perhaps because of—intensified enforcement.
Baltimore's experience mirrors national trends. The city's health department has distributed naloxone (an opioid overdose reversal medication) widely, trained residents in its use, and established supervised consumption sites in some neighborhoods. These harm reduction approaches treat potency and contamination as public health problems rather than moral failures.
What the Data Shows
Research from RAND Corporation and other institutions has documented these trends across multiple datasets. Purity-adjusted prices have fallen for cocaine, heroin, and methamphetamine since 1980. Simultaneously, emergency room visits for drug-related complications have increased, and the pharmacological profile of seized drugs has shifted dramatically toward synthetic substances.
Sample analysis reveals another troubling pattern: unpredictability. Street drugs increasingly contain mixtures of substances, sometimes including multiple synthetic opioids or stimulants. Users cannot reliably know what they're consuming or in what concentration.
Policy Implications
The evidence suggests that enforcement-focused approaches have inadvertently optimized the illicit drug market for potency and efficiency. Each interdiction success creates selection pressure favoring more concentrated, easily concealed substances.
Some jurisdictions have begun exploring alternatives. Portugal decriminalized personal drug possession in 2001, redirecting resources toward treatment and harm reduction. Overdose deaths decreased substantially, though Portugal's smaller scale and different social context limit direct comparisons to the United States.
Canada and some U.S. states have experimented with supervised consumption sites, prescription heroin programs for chronic users, and drug checking services that allow users to test substances for contaminants. These approaches accept that drug use will occur and focus on reducing associated harms rather than eliminating use through prohibition.
The Baltimore experience, as reported by the Times, illustrates a fundamental tension: the tools that seem most intuitive for addressing drug problems—interdiction, criminalization, supply disruption—may actually worsen the specific harms they aim to prevent. Fifty years of data increasingly points toward that uncomfortable conclusion.
Whether policymakers will adjust strategies based on this evidence remains uncertain. But the chemistry is unambiguous: prohibition has created powerful evolutionary pressure toward exactly the kind of drugs that pose the greatest danger.
